  • Sick Leave / Carers Leave: Please be aware that Doctor's Certificates are mandatory for leave lasting more than two days.
  • Compassionate Leave: For immediate family members only.

PROCESS

  1. Please make sure that all mandatory form fields are filled out.
  2. An automatic email notification will be provided to the Office Manager who will handle your request.
  3. A copy of your completed form will be sent to you via email.
  4. You will receive an email notification once your request has been processed.
